As such, this form must be completed in advance of the event you are attending or your appointment on our campus. It must be completed by individual visitors, not as a collective group. We recognize you may be completing this form several days before your visit to CAJ and your health may change between submission of this form and your arrival. Should you begin exhibiting symptoms of illness or fever, we ask that you postpone or cancel your visit to CAJ.
質問用紙に記入された後、実際にCAJに来校して用紙を提出されるまでに、体調が変わってしまう場合もあるかと存じます。記入されたときには何もなくても、その後発熱等病気の症状が現れた場合は、来校を控えていただき、アポイントメントをお取り直しください。

Do not enter, and reschedule your visit, if either of the statements below apply to you:
1) You have returned from a foreign country, or have had significant contact with a traveler who has returned from a foreign country, in the past 14 days.      
2) You have experienced symptoms of persistent cough, fever (>37.4°C), or trouble breathing/shortness of breath within the last 72 hours.
以下の項目に当てはまることが一つでもある場合は、ご来校をお控えいただき、アポイントメントを取り直してください。
１）過去14日間以内に外国から帰国した、または帰国者との濃厚接触があった
２）しつこい咳、熱（37.4度以上）、息切れや呼吸が苦しいなどの症状が過去72時間以内にあった
